Location: Adur NMHT, Glebelands Middle Road, Shoreham By Sea, BN43 6GA, UK

Seeking an experienced, enthusiastic, and motivated Community Support Worker to join a friendly team supporting individuals with severe and enduring mental health problems. The team provides specialised mental health care within a recovery-focused model designed to enable and encourage service users to develop positive coping strategies to maximize their independence and potential.

Duties:

  • Undertake clinical visits to patients and their carers out in the community.
  • Assist community nurses to plan, implement, review, and evaluate individual patient care plans.
  • Work as part of a multi-disciplinary team, feeding back concerns to ensure risks are identified and acted upon.
  • Deliver safe and compassionate care while proactively promoting social inclusion and positive occupation.

Requirements:

  • Good general education along with an NVQ Level 3 or equivalent experience.
  • Experience of working with mental health needs in a paid or voluntary capacity.
  • Experience of working collaboratively with colleagues, wider teams, and service users.
  • An understanding of confidentiality.
  • Subject to a Disclosure and Barring Service (DBS) check.
